Question: How do i make a blog affiliate sign up html code?

I have a website on blogger for Twilight called http:/ / www.edwardcullensjournal.com that I am looking to make an affiliate sign up page for. I am looking to make one similar to this: http:/ / twilightersanonymous.com/ other/ affiliates- sign- up.html But I have very little html coding skills and do not have the first clue as to how to do this and cannot find it online. The way that this website has done it is very complicated and I cannot seem to figure it out for my website. I have downloaded their package to do this, but don't understand the instructions. Can I get either simple instructions for this or another way to make this worksheet for my website? Thanks!

Well the code they used is a php/ database**probably** script Which blogger doesn't allow I couldn't imagine it being done on your blogger page in any other way than making a page titled affiliate or something and tell the details of how to become affiliates and comments will be the ones you go through then manually add in their link or something
